Psychiatric Nursing

The goal ofVisiting Nurses of Meredith & Center Harbor Psychiatric Nurse program is to provide special assessment and teaching for patients with mental/emotional disorders. Patients may be referred by physicians for a psychiatric nursing evaluation in the home by our certified psychiatric nurse.  Within the agency, any discipline may request a psychiatric nursing evaluation for a client with a behavior problem or mental status change.  Each psychiatric client will have an individualized plan of care designed to meet specific goals and outcomes.

The program includes: Comprehensive mental health assessment Multidisciplinary approach to care Medical evaluation and recommendations Home safety evaluation and management Functional assessment Integration of medical conditions
